Overview

A urinary tract infection (UTI) is a microbial infection of any part of the urinary system, comprising the urethra, bladder, ureters, and kidneys, most commonly caused by ascending bacteria such as uropathogenic Escherichia coli. Infections are classified by site as lower tract, including urethritis and cystitis, or upper tract, including pyelonephritis, and as uncomplicated or complicated depending on host factors and structural abnormalities. Typical features include dysuria, urinary frequency and urgency, and cloudy or bloody urine, with systemic signs in upper-tract disease; untreated infection can ascend and cause renal damage or sepsis. Diagnosis relies on urinalysis and culture, and management on appropriate antimicrobial therapy, an effort increasingly complicated by antimicrobial resistance. Research relevant to this area examines the development of multiplex real-time PCR assays for detecting extended-spectrum beta-lactamase genes in urinary tract infections, antibiotic-resistant infection and its management, and resistance patterns in elderly and other populations. Further work addresses the bacteriological quality of water sources, antifungal and antimicrobial susceptibility, and the structure-activity basis of antibacterial agents such as sulfonamides. The field intersects urology, microbiology, and infectious disease, with growing emphasis on resistance. The journal publishes peer-reviewed research on urinary tract infections, including molecular detection of resistance genes, antimicrobial susceptibility, and the management of resistant infection.
